Dear CREC Families,
As per the Connecticut State Law (C.G.S. Sec. 10-204a and 10-206), all children must provide the school with proof of the following requirements in order to attend 7th grade at the start of the 2019-2020 school year.
A complete physical exam by a health provider done on or after July 1st , 2018.
An updated immunization record which includes these vaccines: a 2nd dose of MMR (measles, mumps, rubella), a 2nd dose of varicella (chickenpox) or verification of disease from a health care provider, Tdap booster, and meningococcal.
The form with the physical exam and record of immunizations must be given to the school PRIOR to the 1st day of school. Your child can not attend any CREC unless this information is complete and provided to your child’s school. The physical exam and immunizations should be recorded on the State of Connecticut Department of Education Health Assessment form (HAR). All health care providers have access to this form. We encourage you to call your health care provider now to set up an appointment to have this done as the summer is quite a busy time for them.
CT state law also requires that you have your health care provider fill out the “Authorization for the Administration of Medicine by School Personnel” form if your child is to receive any medication (prescription or non-prescription) while at school. This form can be provided by your health care provider or you may use the form I have enclosed. Please remember that all medication must be delivered to the school by a parent or guardian in the original, unopened container with the child’s name on it.
These forms can be faxed directly to the fax number below or you may deliver them to school. Please do not hesitate to contact the school nurse with any questions or concerns regarding this.
Thank you,
David Cusick
